This eReferrals Services Schedule (\u201cService Schedule\u201d) supplements and is incorporated by reference into the Master Services Agreement <\/span>https:\/\/thinkresearch.com\/ca\/company-overview\/msa<\/span><\/a>. This Service Schedule and its Attachments are effective so long as there is an active Order Form. Unless otherwise defined in this Service Schedule, capitalized terms will have the meaning given to them in the Agreement.<\/span><\/p>\n

  1. Personal Health Information.<\/b>\n
      \n
    1. To facilitate the Licensee\u2019s use of the Licensed Product, Think Research may, as required, act as an \u201cAgent\u201d to the Licensee in the Licensee\u2019s capacity as a Health Information Custodian (\u201cHIC\u201d) as both terms are defined by Ontario\u2019s <\/span>Personal Health Information Protection Act<\/span><\/i> (\u201c<\/span>PHIPA\u201d).  As an Agent, the Think Research will act for or on behalf of the Licensee in respect of the collection, use or disclosure of Personal Health Information (\u201c<\/span>PHI\u201d) as defined by PHIPA, for the purposes of the Licensee and not the Think Research\u2019s own purposes.  As an Agent, Think Research will not disclose PHI to any person other than the Licensee or the Licensee\u2019s authorized employees, contractors and healthcare practitioners (the \u201c<\/span>Authorized Users\u201d).  Think Research will direct all requests for PHI, other than those requests made by Authorized Users, to the Licensee. <\/span><\/li>\n
    2. The Licensee is at all times responsible for ensuring patient consent and authorization for the collection, use and disclosure of PHI under this Agreement.<\/li>\n<\/ol>\n<\/li>\n
    3. Customer hereby acknowledges that in order to deliver certain Services, Think Research may transmit Personal Information or data between Customer and another customer, or between Customer and a patient as part of the intended use of the Services.<\/li>\n<\/ol>\n","protected":false},"excerpt":{"rendered":"
