Collaboration makes the latest clinical knowledge accessible to more physicians and patients to help reduce readmission rates<\/h3>\n\n\n\n

(Toronto, ON) May 6, 2020<\/strong> \u2014 An innovative new partnership between the Canadian Heart Failure Society (CHFS) and Think Research, a health technology and clinical content company, is bringing together new research and clinical tools \u2014 making them more accessible to those who treat people with heart failure across Canada. 
<\/p>\n\n\n\n

Through the project, CHFS has made recommendations for an updated heart failure Order Set. Developed in collaboration with Think Research\u2019s in-house clinical team, it aligns to the most up-to-date, evidence-based research. The goal of this order set is to prevent heart failure-related hospital readmissions and improve patient outcomes by facilitating use of guideline directed medical therapy.
<\/p>\n\n\n\n

Heart failure affects well over half a million Canadians. During the COVID-19 pandemic, vigilance around heart health is proving incredibly crucial. 
<\/p>\n\n\n\n

Heart failure continues to be the second most common reason that Canadians who are 65 and older end up in hospital. In addition, within 30 days of discharge, 21 percent of heart failure patients are readmitted for follow-up care. Mortality rates are also high. 
<\/p>\n\n\n\n

\u201cThere has been a great deal of research and advances in how we treat people with heart failure,\u201d says Dr. Shelley Zieroth<\/strong>, CHFS president. \u201cAn important part of our mandate is to ensure our hospital-based colleagues across Canada have the latest evidence-based recommendations to optimize treatment for their patients. There\u2019s considerable evidence that the patient\u2019s hospital admission is a critical inflection point. We\u2019re excited to partner with Think Research to help standardize how we care for heart failure in this country.\u201d
<\/p>\n\n\n\n

By 2030, heart failure care is estimated to cost more than $2.8 billion per year in Canada. Much of this cost can be attributed to stubbornly high hospital readmission rates. Standardized clinical orders aim to improve action and awareness on this issue.
<\/p>\n\n\n\n

Canadian hospitals within Think Research network\u2019s have access to the updated CHFS Heart Failure Order set as of May 4, 2020. Hospitals outside the network can still access the Order Set by registering at hfordersets.ca<\/a>.<\/p>\n\n\n\n
